Please don't use behavior charts like these. It is an outdated concept. No adult would tolerate having every mistake made visible to their coworkers. We shouldn't be publicly shaming kids either. Correct behavior and teach expectations - but don't put mistakes on display. For many kids, this leads to worse behavior because they're embarrassed.
