Has both gaskets and necessary screws. If your valve has hot and cold shut-offs (mine were built into the valve) it's a medium do it yourself job. Several older manufacturers used this. Mine was installed in 1991. 
Take pictures while taking apart. Because it was used by many manufacturers, the assembly or your brass valve parts will vary.
